Outreach Case Manager - Compensation & Benefits$22.00 - $26.00 per hour, depending on experienceFull-time positionVision, dental, and medical insurance offered through O2 Employment ServicesOpportunity to work in a meaningful, community-focused role helping underserved populations throughout the North State Outreach Case Manager - QualificationsHigh school diploma or equivalent requiredAssociate's degree in Social Work, Human Services, or related field preferredPrevious experience in community services, case management, outreach, housing navigation, or related field preferredFamiliarity with HIPAA, Title 42, billing processes, and documentation standards preferredStrong interpersonal and communication skillsExcellent organizational and time-management abilitiesProficient with Microsoft Office and client management softwareAbility to work effectively with diverse populations in challenging environmentsMust have a clean DMV record and be insurable through company auto insurance providerAbility to maintain professionalism and composure in high-stress situations Outreach Case Manager - ResponsibilitiesConduct client intake interviews to determine eligibility for ECM and Community Support servicesAssist clients with enrollment paperwork and provide education regarding available programs and servicesMaintain accurate client documentation and ensure compliance with HIPAA and confidentiality standardsTrack client outcomes, quality performance indicators, and program effectivenessParticipate in HMIS data entry, assessments, housing screenings, and case documentationAssist clients with housing navigation, applications, eligibility verification, and obtaining required documentationCoordinate with landlords, housing providers, healthcare systems, emergency personnel, and community organizationsConduct outreach in rural, urban, and encampment settings to engage underserved populationsBuild rapport with clients using trauma-informed care principlesSupport clients experiencing housing instability, substance use disorders, mental health crises, and other barriers to careAttend meetings and collaborate with internal departments and external agencies to support coordinated care effortsRespond appropriately to crisis situations while maintaining professional boundaries and safety protocolsOther duties as assigned Outreach Case Manager - Work EnvironmentExtensive field-based work and community outreachOutdoor work in varying weather and environmental conditionsFrequent interaction with individuals experiencing homelessness, mental health conditions, substance use disorders, and crisis situationsCollaboration with healthcare providers, emergency response personnel, housing providers, and community partners Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
