DIETITIAN/FAMILY PRACTICE - Part-time (Fridays, 9:00a-2:00p)

The Toledo Clinic is seeking a part-time Weight Management Registered Dietitian to join our Timberstone Family Practice. This position will be scheduled on Fridays from 9:00am - 2:00pm. The office is located at 4411 Holland-Sylvania Rd in Toledo OH.General Summary:The weight management Registered Dietitian supports the primary care physician/ referring provider by providing nutritional assessment, education and management to patients with chronic conditions who are at risk for disease progression and complications. In collaboration with the physician and interdisciplinary team, the Registered Dietician develops and implements an individualized care plan designed to promote the patient's understanding and management of their condition, optimizes quality of life, and improves clinical outcomes. The Registered Dietitian will also help to develop, implement, and assess nutritional programs for the weight management program.Principal Duties & Responsibilities:Example of Essential Duties:Conduct comprehensive nutritional assessment, education, and management to patients referred by physicians and/or multidiscipline care teamDevelop, implement, and coordinate individualized meal plansReview and analyze patient's nutritional needs and goals to make appropriate dietary recommendationsAssess patients' knowledge and tailor education to fit patients' needsProvide outpatient education and counseling to patients and families in individual or group situations, in accordance with established educational plansCollaborate with multidisciplinary team and community professionals to ensure continuity of careCommunicate regularly with weight management program coordinatorMaintain timely clinical documentation in compliance with the Toledo Clinic, Inc. policies and regulatory standardsObtain appropriate insurance information and properly code services providedMaintain appropriate dietary status information on patientsDevelop patient education resourcesRefer patient to appropriate health care provider to assist patient with needs outside of the dietitian's scope of practiceMaintain at all times the confidentiality of the Toledo Clinic, Inc. proprietary information and patient medical, financial, or personal information, and generally any company information not made available to the publicMaintain at all times the confidentiality of former or current Toledo Clinic, Inc. patients' protected health information, as directed by state and federal law, and the Toledo Clinic, Inc. policyOther duties as assigned.Knowledge, Skills & Abilities:Required:Consistently arrives at work, in professional attire, on time and completes all tasks within established time frame.Seeks appropriate tasks when primary tasks are completed and assists co-workers as needed.Demonstrates adaptability to expanded roles.Must be a licensed Dietitian in the state of OhioAbility to coordinate with other healthcare professionals and staffMust have exceptional problem-solving, critical-thinking, organizational, interpersonal, and written/verbal communication skillsWorking knowledge of computer programs (email, Word, Excel, PowerPoint, etc.)Willingness to comply fully with all organizational and departmental policies and procedures.Preferred:Experience in diabetes education and/or CDCESExperience in weight loss clinicRDN is strongly preferredEducation:* Bachelor of Science in Nutrition/ Dietetics with a current registration by the Commission on Dietetic Registration
