Licensed Social Worker
Collaborates with physicians and clinical staff in initiation, development, coordination and integration of ongoing clinical assessment, patient care planning, patient teaching, assessment of patient progress through the continuum of care and evaluation of discharge requirements necessary for the achievement of determined outcomes. This responsibility is assumed within the context of appropriate allocation and utilization of resources and quality of care, collaborating with professional and community disciplines to support the patient along the continuum of care.
Functions as liaison internally and externally. Is knowledgeable of all local community, city, and state resources and serves as a resource for others. Makes appropriate referrals to local community, city, or state agencies for services needed, serving as a patient advocate and maintaining patient rights and safety. Provides referrals for services indicated by screening and assessment. Serves as a resource to in-house patients and families, interdisciplinary staff, the community, and physicians, and participates in committees as appropriate. Bachelor’s degree or higher preferred.
Experience in a medical office or urgent care and emergency room setting preferred.
Knowledge of various software applications, i.e., Microsoft Office, including Excel and PowerPoint preferred.
If a Licensed LPN or RN, prefer that employee have clinical experience.
The employee must have strong interpersonal communication skills with an ability to work independently, problem solve, and be able to resolve conflict in an assertive manner.
The employee must have strong organizational and time management skills as evidenced by a capacity to prioritize multiple tasks and role components.
