Job Description PRN Physical Therapist (PT) – Liberty, TXLiberty Health Care CenterLiberty Health Care Center is seeking a PRN Physical Therapist (PT) to join our in-house rehabilitation team in Liberty, TX. We are committed to providing high-quality, patient-centered care in a skilled nursing setting, helping residents improve mobility, strength, and overall functional independence.Our in-house therapy program promotes collaboration, continuity of care, and strong interdisciplinary teamwork.Position SummaryAs a Physical Therapist (PT), you will be responsible for evaluating residents, developing individualized treatment plans, and delivering skilled therapy services to maximize functional outcomes and quality of life.Key Responsibilities• Perform physical therapy evaluations and establish individualized plans of care• Provide skilled PT services in accordance with physician orders and clinical standards• Address mobility, balance, strength, gait training, and pain management• Monitor resident progress and modify treatment plans as appropriate• Educate residents and caregivers on exercises, safety, and functional mobility• Complete accurate and timely documentation per facility, state, and federal guidelines• Collaborate with the interdisciplinary team to support optimal resident outcomes• Maintain compliance with all professional, ethical, and facility standardsQualifications• Active Texas Physical Therapist (PT) license• In good standing with all regulatory and licensing boards• SNF or long-term care experience preferred• Familiarity with Net Health is a plus• Strong clinical, communication, and time-management skillsWhy Join Liberty Health Care Center?• In-House Therapy Program• Flexible PRN Schedule• Supportive and Collaborative Work Environment• Opportunity to Make a Direct Impact on Resident CareConfidential inquiries are welcome.
