As our Adult Services Program continues to grow, we are looking for a motivated, compassionate Adult Targeted Case Manager who wants to make a meaningful difference in the lives of adults living with serious and persistent mental illness (SPMI). If you are someone who believes in empowering individuals, coordinating meaningful supports, and helping people build stability and independence in their communities — this role is for you.What You'll DoAs an Adult Targeted Case Manager, you will work closely with clients to connect them to the services and supports they need to thrive.
