CarePlus TLC is seeking a dedicated, compassionate, CNA/Home Health Aid with a passion for serving others to join our team! This role requires a commitment to delivering high-quality care while maintaining a clean and safe environment for clients. The successful candidate is self-motivated, reliable, compassionate, and committed to creating a positive experience in our client's livesOur staff create a friendly, caring environment to ensure our clients receive the quality care and assistance they need to maintain their independence and quality of life in their home. Clients is located in Nunica. Husband and Wife duo. Both have 2 hours 2 days per week on Tuesdays/Thursdays. They would like shifts together for a 4 hour block. 8 hours per week with this case. Hours are 11am-3pmResponsibilities:Monitor patient's condition and report changes/concerns to appropriate medical professionals and family membersProvide Activities of Daily Living care as outlined in the Care Plan-bathing, dressing, feeding, etc.Meal prepTransfers patient with equipment using appropriate safety measures (gait belt, hoyer, sit-to-stand lifts)Perform light housekeeping duties to maintain a clean-living space for clients. Ability to turn and reposition patientsUnderstands First Aid and Emergency Care protocolsComplete electronic documentation while on shiftMedication remindersSkills:Familiarity with Home Health Care, safety and sanitation standards and proceduresUnderstanding of Confidentiality and Privacy LawsExcellent communication and interpersonal skills… able to communicate effectively with patients and their families.
