The Social Work Program Coordinator is responsible for the professional and administrative management of the Veteran Directed Care (VDC) within the NAVAHCS system. The incumbent manages and controls the use of data related to all programs regarding day to day functioning and program evaluation.DutiesThis is an OPEN CONTINUOUS ANNOUNCEMENT and may be posted up to July 15 - and implementation of the project componentMajor duties and responsibilities of the positionAdministrative Provides administrative - technical - budgetary monitoring and oversight necessary for accomplishing the program coordination and management functions for the VDC programPerforms a wide range of services and provides technical assistance to State - and implementation of the project componentInitiates consultation with social work service staff as well as GEC staff and possibly with other authoritative resources at the Medical CenterProgram Coordination Serves as the Social Work Coordinator of the VDC programDuties include all administrative oversight and clinical responsibility for all programs under responsibility and is responsible for weekly - monthly - and quarterly meetings and reports respective to each assigned programResponsible for program reports to VISN and VACO for the coordinated programsDuties include ensuring Medical Center Memorandum compliance with the VDC programAs the Coordinator - incumbent is responsible for scheduling - organizing and inspecting of community provider facilities of each program respectively and completion of appropriate required reports and follow-up to ensure program operationResponsible for negotiating - facilitating and completing basic ordering agreements/contracts for the VDCResponsible for the development - coordination - selection and evaluation of VDC - implementation and application of VA guidelines to ensure standards are maintainedThe VDC Coordinator will be the primary Contracting Officer's Representative - level 2 (COR) assigned to oversee existing nursing home contracts - and the acquisition of new contractsMaintains an unrestricted - unsuspended license to practice social workMust be able to provide supervision for licensure Other duties as assigned Work Schedule: Monday -
