LVN FIELD HOSPICE NURSE

Overview We are seeking a compassionate and dedicated Hospice Nurse to join our team. The ideal candidate will provide high-quality nursing care to patients in a hospice setting, focusing on comfort and quality of life for those with terminal illnesses. This role requires a strong commitment to patient-centered care, effective communication skills, and the ability to work collaboratively with interdisciplinary teams. Responsibilities Deliver comprehensive nursing care to hospice patients, ensuring their comfort and dignity. Conduct thorough assessments of patients’ physical, emotional, and spiritual needs. Develop and implement individualized care plans in collaboration with the healthcare team. Administer medications and treatments as prescribed, including botulinum toxin treatment when necessary. Provide education and support to patients and their families regarding end-of-life care options. Monitor patient conditions and report any changes to the healthcare team promptly. Ensure compliance with HIPAA regulations and maintain patient confidentiality at all times. Participate in case management activities to coordinate care effectively. Document all patient interactions accurately in the electronic health record system (HOSPICEMD). Skills Strong case management skills with the ability to navigate complex healthcare needs.
