There is no on-call requirement. Join our Pediatric Homecare Team and experience nursing the way it was meant to be personal, meaningful, and deeply rewarding.What You’ll Do as a Pediatric RN Case ManagerProvide high-quality nursing care to children and support families through a mix of hands-on clinical care, assessment, education, and care coordination. You’ll be trusted, valued, and empowered to use your clinical judgment to make a meaningful difference.Your Responsibilities IncludeDeliver safe, family-centered pediatric nursing care in the homeServe as the primary case manager, coordinating care with the interdisciplinary teamDevelop and implement individualized plans of careProvide family teaching, support, and educationEnsure accurate, timely documentation (Documentation time built into the home visit)Supervise and provide oversight of paraprofessional staff, including home health aides, ensuring tasks are performed safely and according to the patient’s plan of care.Maintain compliance with all infection control and regulatory standardsStay current with clinical best practicesWhat We’re Looking For!Current NYS Registered Nurse license (accredited program)BSN preferred (tuition reimbursement available!)Valid driver’s license, auto insurance, and reliable vehicleSelf-directed, organized, and comfortable working independentlyPediatric or maternal/child health experience a plusStrong communication and computer skillsPhysical RequirementsAbility to sit or stand for long periodsAbility to lift/carry 25–40 lbs and assist with transfers up to 150 lbsComfortable with bending, kneeling, climbing stairs, and driving in all weatherAbility to use a phone, laptop, tablet, and electronic medical recordCategory III bloodborne pathogen exposure (rare contact with blood/body fluids)Compensation & BenefitsCompetitive Salary [This position is an S03 exempt position with a min-max rate of: $75,250- $87,138.92/annually] with:$15,000 sign-on bonus*Retention BonusesInternal Mentoring Program401K with generous Employer matchOn-Site Gym (free for all employees)Excellent work/life balance (no rotating shifts, only 2 required holidays per year on a rotating schedule)Medical, Dental, Vision plansTuition Reimbursement (BSN after only 6 months of employment!)Partially funded HSAEmployee Recognition PlatformPaid Time Off, Holidays, Sick and Extended Sick LeaveShort/Long term DisabilityEmployee Assistance Program (EAP)Much More!________________________________________________About Nascentia HealthNascentia Health is leading the way in home care, post acute care and long-term community health.
