You will provide care coordination and behavioral health support to patients, caregivers, and their loved ones while implementing Synapticure’s Collaborative Care program. This role involves working closely with medical providers, patients, and partners to drive meaningful change and ensure exceptional mental health care for this community.Job Duties – What You’ll Be DoingSupport the mental and physical health care of assigned patients, ensuring comprehensive coordination with their medical providers and other mental health professionalsScreen and assess patients for mental health and substance use disorders, facilitating engagement and follow-up careProvide education to patients and caregivers about mental health and substance use disorders, including available treatment optionsMonitor clinical symptoms and treatment outcomes, documenting progress and adapting care plans as neededDeliver evidence-based behavioral interventions, including techniques such as motivational interviewing, behavioral activation, and problem-solving treatmentsCollaborate with psychiatric consultants to develop and adjust treatment plans for patients who are not improving as expectedFacilitate referrals to external services, such as social services, vocational rehabilitation, and specialty care, as clinically indicatedMaintain accurate records in Synapticure’s EHR system, ensuring clear communication with all care team membersDevelop and implement relapse prevention plans with patients achieving treatment goalsBuild relationships with patients, caregivers, and partners to foster trust and ensure continuity of careRequirements – What We Look For In YouActive RN or LCSW licensureMinimum of 2 years’ experience in direct case or care managementBackground in ALS, Parkinson’s Disease, or other neurodegenerative or chronic disease careStrong communication and interpersonal skillsFamiliarity with insurance processes and medical billing/codingExperience with reviewing medical records and working within clinical trialsProven ability to thrive in a startup or rapidly growing organizationKnowledge of current trends in neurodegenerative diseases, terminal illnesses, targeted therapies, and clinical trialsPassion for working with vulnerable populations and providing patient-centered careWe’re founded by a patient and caregiver, and we’re a remote-first company.
