If you share our commitment to delivering exceptional patient care and are passionate about making a difference in people's lives, we encourage you to apply for this rewarding opportunity. **Responsibilities** - Provide direct care and therapy to residents with a range of physical and cognitive abilities, including those with memory loss, dementia, and other chronic conditions. - Develop and implement individualized care plans that align with each resident's unique needs and goals. - Conduct thorough assessments to identify areas of strength and weakness, and develop strategies to improve mobility, balance, and overall physical function. - Utilize various treatment modalities, such as physical therapy, occupational therapy, and speech therapy, to promote rehabilitation and independence. - Collaborate with other healthcare professionals, including nurses, occupational therapists, and social workers, to ensure comprehensive care. - Monitor resident progress and adjust treatment plans as needed to ensure optimal outcomes. - Educate residents, families, and caregivers on therapeutic exercises, home safety, and fall prevention techniques. - Maintain accurate and detailed records of resident care and therapy sessions. **Skills** - Experience in caregiving and providing direct support to individuals with a range of abilities and needs. - Familiarity with home community care and long-term care settings. - Strong background in behavior management and the ability to adapt to diverse resident needs. - Proven ability to work effectively in assisted living, memory care, and nursing home settings. - Proficiency with various systems and electronic medical records (EMRs). - A passion for delivering high-quality care to individuals in need. - A commitment to the principles of caregiving, including empathy, compassion, and respect for each resident's autonomy and dignity. - Excellent communication and interpersonal skills, with the ability to work collaboratively with other healthcare professionals and build strong relationships with residents and families.
