LTSS Intern

Assists care managers with providing care management for managed care enrollees in the LTSS divisionEvaluate assessment and care management data to determine ongoing program eligibility, care needs and service options. Understand how to make Level of Care determinations for consumers as part of initial, annual and event-base (if applicable) assessment processMonitor and evaluate the care plan, service quality and program eligibility Collaborate with the Department of Job and Family Services to establish and/or maintain Medicaid eligibilityAssist with completing the enrollment process of eligible consumers into the program Provide education and support to consumers and their caregiversUnderstand how to develop and implement an individualized care plan for all consumersAdvocate on behalf of consumer and/or caregiver/family and assist individuals in securing appropriate services and care settings Assist with disenrolling ineligible consumers from the program Apply judgement and perform clinical practice based upon education, experience and professional practice standards. Participate in and contribute towards accomplishment of divisional and Agency dashboard goalsInterested in developing an understanding of care management roles and responsibilities Interest in working with the elderly and disabled populationPhysical/Mental Position QualificationsAbility to effectively communicate with the elderly and disabled population and general public, both in person and by phone,Ability to comprehend written material related to essential job functions,Ability to accurately document facts related to essential job functions,Ability to operate a computer well enough to perform essential job functions,Ability to comprehend, recall, and apply facts related to essential job functions,Ability to analyze, evaluate, and implement a reasonable course of action based on available information,Ability to detect specific odors related to the assessment of a client's health/hygiene and the safety of the client's environment,Ability to legally drive a car or make other transportation arrangements, andAbility to ambulate enough to access private homes/locations which may not be readily accessible.
