This role focuses on delivering high-quality direct patient care and rehabilitation support within a structured skilled nursing environment.The ideal candidate will have experience assisting residents with daily living activities while ensuring safety, dignity, and comfort. This position works closely with nursing and rehabilitation teams to support patient recovery and overall well-being.Key ResponsibilitiesAssist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and feedingProvide safe mobility assistance and patient transfersMonitor, record, and report vital signs and patient condition changesSupport nursing staff with routine patient care tasksEnsure patient safety, comfort, and dignity at all timesAssist with rehabilitation-focused care activities as directedMaintain accurate and timely documentation of patient care providedFollow infection control protocols and facility safety standardsCommunicate effectively with interdisciplinary care teamsRespond promptly to call lights and patient needsRequired QualificationsActive Certified Nursing Assistant (CNA) certificationPrior experience in long-term care or rehabilitation setting preferredStrong patient care and communication skillsAbility to assist with ADLs and mobility supportKnowledge of infection control and safety proceduresAbility to work effectively in a rehabilitation healthcare environmentFor more details contact at hdavda@navitashealth.com or Call / Text at 516-862-1169.
