A company is looking for a Transition Specialist to support the coordination of member transitions in the community. Key Responsibilities Support care coordination teams in developing effective transition plans for members Assist with member transitions based on enrollment or care services identified Track and maintain transition metrics to identify trends and process improvements Required Qualifications Bachelor's degree and 2 - 4 years of related experience, or equivalent experience California LVN/LPN State Licensure required Must be licensed in California Prefer candidates to reside in California Availability to work PST hours
