You will provide comprehensive physical therapy services to adults across a variety of clinical conditions—delivered in patients’ homes or community settings throughoutNaples, Bonita Springs, Fort Myers, and Marco Island . You’ll manage your own caseload, collaborate with a multidisciplinary team, and implement interventions that support real-life functional carryover.Key Responsibilities:
Conduct initial evaluations, ongoing assessments, and discharge planning
Develop and implement individualized, evidence-based treatment plans focused on mobility, balance, neurological rehab (including Parkinson’s), post-surgical rehab, postural/body mechanics training, home safety assessments, and lymphedema management
Provide one-on-one therapy with a hands-on, patient-centered approach and strong focus on education and functional outcomes
Monitor, document, and report patient progress while meeting compliance requirements and securing necessary authorizations
Collaborate with the clinical team to ensure optimal continuity of care and patient success
Travel to patient homes within the assigned region; maintain necessary clinical equipment and suppliesCompensation, Benefits & What Makes This Role Special

Qualifications
Active Florida Physical Therapist license (or eligibility for licensure)
Experience in outpatient, home-based, or mobile therapy preferred
Strong assessment and clinical reasoning skills; experience with post-surgical rehab, neurological conditions, balance/mobility disorders, and/or lymphedema is a plus
Comfortable working in a mobile care model and traveling between client homes (vehicle or travel stipend may be provided)
Excellent communication and interpersonal skills with the ability to build rapport and empower patients in their home environment
Self-motivated, organized, and able to manage a caseload while maintaining high standards of care and documentation
Proficient with technology for scheduling, EMR documentation, and virtual collaboration
