The Licensed Practical Nurse is responsible for providing direct patient care under the supervision of the Registered Nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient outcomes.Essential Job Functions: Provides direct patient care within the parameters of current nursing license. Implements current nursing practice by following the patient assessment and approved plan of care, which was initiated by the Primary Registered Nurse. Complete, maintain and submit accurate and relevant clinical notes regarding patient's condition and care. Record pain and symptom management changes and/or outcomes as appropriate. Point of Service documentation is an expectation of all clinical personnel for quality care, accuracy, and communication standards. The clinician is expected to appropriately document at Point of Service, and effectively communicate with the Patient Care Team. Provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care. Participates in the coordination of Home Health services, appropriately reporting the identified needs to RN Uses and prepares equipment and supplies effectively, adhering to aseptic technique. Participates in personal and professional growth and development. Participates in the organization's quality management program. Other duties as assigned.Qualifications (Minimum qualifications will be considered required unless specifically stated otherwise) Experience: Minimum of one (1) to two (2) year's experience in health care. Three (3) years experience in health care preferred. Experience in a home health care preferred. Experience in acute care in an institutional setting preferred. Experience in homecare preferred. Education and Training: Graduate of an accredited Practical Nurse or Vocational Nursing program. License, Certification & Registration: Possesses and maintains current CPR Certification. Current Massachusetts licensure: LPN/LVN Valid driver's license and automobile insurance Other Requirements: Demonstrates good verbal and written communication, and organization skills. Strong observation skills and problem solving skills. Working Conditions May be exposed to extremes of heat and cold in all weather conditions. Must drive in various weather conditions on roads in varying repair. May be exposed to infections and contagious diseases. Contact with patients under a wide variety of circumstances. May be exposed to/occasionally exposed to patient elements. Subject to varying and unpredictable situations. Handles emergency or crisis situations. Travel required. OSHA exposure category: OSHA exposure category: Category I Category I - Position includes tasks that involve exposure to blood, body fluids, tissues. Category II -Position includes tasks that involve no exposure to blood, body fluids, tissues, but employment may require unplanned Category I tasks. Category III - Position includes tasks that involve no exposure to blood, body fluids, tissues; would not be required to perform Category I tasks.
