As a Healthcare Advocate with Clergy Care, you will serve as a trusted companion and guide for the clergy of the Archdiocese of Philadelphia, helping them navigate a healthcare system that can be overwhelming, confusing, and impersonal.This is not a bedside role. There are no night shifts, no patient ratios, no charting marathons. Every day." THE LICENSE QUESTION Your active license is NOT required. RNs and LPNs with lapsed or inactive licenses are warmly encouraged to apply. CNAs and Medical Assistants with 3+ years of healthcare experience are also welcome. WHAT YOU WILL DOYour day-to-dayYou will be the single, dedicated point of contact for clergy members in your care. You will know their names, their history, their preferences — and they will know yours. Your work spans four core areas:RELATIONSHIP BUILDING & OUTREACH Build trusted, personal relationships with clergy through regular outreach and on-site visitsServe as a primary point of contact for healthcare navigation, benefits questions, and wellness needsProactively identify unmet needs and connect members with services before problems escalateProvide health education and preventive wellness resources tailored to each memberCARE COORDINATION & NAVIGATION Coordinate medical, dental, and specialist appointments for seamless care experiencesAssist members in navigating provider networks, benefits, and pre-authorization processesConnect members with community resources including transportation, home care, and support servicesCoordinate third-party services such as DME, home health, and specialty providersADVOCACY & HOSPITAL SUPPORT Conduct on-site hospital visits and serve as a personal advocate during hospitalizationsProvide post-discharge follow-up and care coordination to prevent readmissionsVisit members in assisted living facilities, skilled nursing facilities, and retirement homesConduct home safety assessments and arrange modifications or meal services when neededAssist with remote monitoring tools including glucometers, blood pressure cuffs, and alert systemsTEAM COLLABORATION Collaborate closely with our broader support team: pharmacists, nutritionists, and social workersPartner with Archdiocesan leadership and affiliated ministries to ensure coordinated, comprehensive careMaintain accurate, timely documentation in accordance with HIPAA and PHI guidelinesWHO WE ARE LOOKING FORThe right person for this roleYou do not need to be currently licensed. You do need to be the kind of person who genuinely loves people, listens well, and finds deep satisfaction in being the one someone can count on.
