Responsible for medication administration, monitoring resident conditions, assisting with daily care needs, and supporting individualized care plans under RN or physician supervision.Roles & Responsibilities:Assess and monitor residents' health status and vital signsAdminister medications and treatments as prescribedProvide wound care and basic nursing proceduresAssist residents with activities of daily living (ADLs)Document patient care accurately in EMR/EHR systemsReport changes in resident condition to RN, physician, and care teamEnsure infection control and patient safety standards are followedEducate residents and families on care plans and treatmentsRespond to emergencies and provide immediate nursing interventionsCollaborate with interdisciplinary teams to support resident-centered care planningRequired Qualifications:Active LPN license of WI or Multistate.BLS required.Minimum 1 year of experienceMeditech experienceAbout MedCadreAt MedCadre, headquartered in Irvine, California and serving healthcare organizations nationwide, we connect exceptional clinicians with world-class career opportunities while upholding the highest standards of ethics, fairness, and integrity.
