Allegria Nursing and Rehab - Position: Director of Social WorkLocation: Port Jefferson, New YorkShift: Full-Time | Monday–Friday | 9:00 AM – 5:00 PMSalary: $60,000 – $72,000 flexible annuallyJob DescriptionAllegria Nursing and Rehabilitation Center of Port Jefferson is seeking a compassionate and experienced Long-Term Care Social Worker Director to join our dedicated family style team. The ideal candidate will provide comprehensive psychosocial support to residents and families while ensuring adherence to state, federal, and facility standards.Key ResponsibilitiesConduct psychosocial assessments upon admission and update as needed.Identify residents' emotional, social, and behavioral needs to create individualized interventions.Provide supportive counseling for residents and families experiencing adjustment challenges, grief, end-of-life concerns, or behavioral issues.Advocate for residents' rights, ensuring dignity, confidentiality, and person-centered care.Participate in interdisciplinary care plan meetings and support goal development.Document assessments, care plans, progress notes, and significant changes per regulatory requirements.Ensure resident preferences and goals are integrated into individualized care plans.Coordinate psychosocial components of admissions and provide facility orientation to new residents/families.Facilitate safe, person-centered discharge planning, connecting residents with community resources as needed.Support residents and families during transfers or care transitions, ensuring communication and emotional support.Serve as liaison between residents, families, staff, and outside agencies.Provide education to families regarding care needs, resources, and facility processes.Lead or participate in family meetings, care conferences, and support sessions.Adhere to federal, state, and local regulations, including CMS and long-term care guidelines.Maintain knowledge of elder abuse reporting requirements, resident rights, and facility policies.Participate in facility quality improvement initiatives to enhance resident outcomes.Candidate RequirementsLicensed Master Social Worker (LMSW) or MSW requiredMinimum 2-3 years of nursing home/long-term care experienceExcellent communication and interpersonal skillsStrong documentation, organizational, and counseling abilitiesAbility to work collaboratively with an interdisciplinary teamKnowledge of community resources and discharge planningFamiliarity with Point Click Care EMR preferred; basic computer skills requiredCompassionate, patient, resident-centered care approachBenefitsComprehensive Medical & Dental CoverageGenerous Paid Time Off (PTO)401(k) Retirement PlanEmployee RecognitionSupportive, family-oriented workplace
