I am aware that I cannot make additional requests after submission.Additional Confirmations I have not requested more than one holiday off during my assignment.Additional Confirmations I have experience at large facilities (300 and up), and at teaching facilities.Additional Confirmations Requires trauma level 1 experienceAdditional Confirmations Willingness to float to units appropriate for skillset is required.Additional Confirmations Experience at a large academic setting (teaching hospital) is required.COVID Vaccine Regular testing required.Flu Vaccine (Current Season) Must be for the current flu season. Medical and religious declinations are accepted. Willing to obtain allowed.Travel Contract Must travel at least 60.0 miles to receive a travel stipend.
