Works collaboratively with the interdisciplinary team to address psychosocial needs and ensure smooth transitions of care.Key ResponsibilitiesCoordinate discharge planning from admission through dischargeComplete psychosocial and behavioral assessmentsDevelop and implement individualized discharge plansCollaborate with nursing, therapy, physicians, and case managementEducate residents and families on post-discharge options and resourcesAddress barriers to discharge, including social, emotional, and mental health needsEnsure compliance with state, federal, and Medicare discharge regulationsMaintain accurate documentation in the medical recordQualificationsBSW/MSW or Psychology degree requiredMichigan licensure or eligibility requiredSNF, hospital, or post-acute experience preferred Powered by JazzHR
