Must have reliable transportation. Must possess a valid driver's license and agency-required liability insurance. Must be willing to travel up to 75 miles from home base. Description: The Physical Therapist is responsible for providing home health visits to patients. They will conduct periodic office meetings as required. The average number of patients seen per day is 5-10, equating to roughly 6-8 points. Contractors are assigned between 5-10 patients per day. Each type of visit and activity, such as documentation and travel, holds a different point value. The expectation is to achieve a weekly productivity goal of 30 points. Patients will primarily be seen in their homes, with occasional visits to care facilities. The position may begin as a contract assignment, with potential for permanent placement. On-call duties may be required every 5-6 weeks. Complete documentation within 24 hours of the patient visit is mandatory. There are no guaranteed hours, and clinicians must be prepared for fluctuating patient census. Travel reimbursement will be provided at the IRS standard mileage rate.
