Speech-Language Pathologist — Bronson At HomeBronson At Home · PRN · First Shift (10 hrs/week) CCC RequiredRole SummaryThe Speech-Language Pathologist evaluates, plans, and provides treatment to patients in the home health setting to facilitate rehabilitation of speech, language, cognitive, communication, and swallowing disorders. The goal is to enhance each patient's functional independence through individualized, evidence-based care across all age groups and populations.Clinical ResponsibilitiesEvaluation & Assessment — Interpret referrals, assess patient functional status through standardized testing, and establish a functional baseline to track progressTreatment Planning — Develop individualized treatment plans based on each patient's age, developmental status, cognitive ability, learning style, cultural/language background, and clinical needsDirect Patient Care — Evaluate and treat speech, language, cognitive, communication, and swallowing disorders; perform hearing screenings as part of the evaluation process; conduct video swallow studies if applicablePatient & Family Education — Educate patients and families on treatment goals, home programs, and strategies to support ongoing progressGoal Setting & Plan of Care — Establish patient care goals, set therapy duration, and adapt treatment plans based on progressInterdisciplinary Collaboration — Participate in interdisciplinary discussions and rounds; serve as a resource in areas of expertiseCoverage — May cross-train and provide coverage at other departments within Rehabilitation Services as neededAdministrative ResponsibilitiesDocument evaluations, treatment plans, treatments performed, and patient progress in the electronic health record per compliance timelinesParticipate in department process improvement initiatives and serve on committees and task forcesProvide in-services in areas of expertise for Bronson and the communitySupervise students and volunteers as neededMaintain and upgrade clinical competencies based on population served, care provided, and advances in healthcareRequired QualificationsEducation: Master's degree in Communication Disorders from an accredited Speech-Language Pathology program with ESB certificationLicensure: Current Speech-Language Pathology license in good standing in the state of MichiganCertification: Certificate of Clinical Competence (CCC) from ASHA requiredBLS: Required within 3 months of hireDriver's License: Valid Michigan driver's license with daily access to a vehicle for business useAuto Insurance: Proof of valid auto insurance on the vehicle used for Bronson business; employee is responsible for submitting renewals and updating coverage if the vehicle changesMVR Check: Must successfully pass a motor vehicle record check after offer of employment, prior to start datePreferred QualificationsPrevious clinical experienceSkills & CompetenciesStrong communication and teamwork skills — ability to work effectively with patients, families, and all members of the care delivery teamEffective time management and organizational skillsProficiency with electronic health records and basic computer/mobile device useSocial sensitivity and the ability to communicate in English effectivelyPhysical Demands & Work EnvironmentConsiderable standing and walking; regular lifting of light objects (20 lbs or less) with occasional heavier tasks including assisting with lifting and moving patientsComputer use up to 40% of the time requiring sustained concentrationHome health setting requires daily driving to patient homesAdditional NotesSome work locations may require a background check and fingerprintingSome locations may require weekend and/or holiday rotation
