Care Navigation Coordinator
Description:
Zepf Center has been serving the Lucas County community for over 50 years. We are the leading provider of behavioral health and substance use disorder services in Northwest Ohio. Services include adult and child psychiatric, substance abuse, case management, residential, Crisis Care, and therapy programs, as well as career development and wellness services. Zepf Center also offers primary care medical services to our patients to contribute to their continuum of care. Zepf Center is a trauma-informed agency and environment for both patients and staff.
Essential Duties and Responsibilities:
Provide patient education and treatment access for patients using SBIRT (Screening, Brief Intervention and Referral to Treatment model)
Maintain internal and external contacts including community-based organizations to help support diverse patient needs
Serve as the liaison between the hospital and the Zepf Center for both the patient and the baby
Coordinate multidisciplinary team meetings that support patient needs, and develop a plan of care that best serves the needs of the individual patient
Assess, link, monitor, advocate and case plan for the patient
Knowledge of current federal, state and county reporting guidelines for substance-exposed infants
Match treatment response to each woman’s’ stage of recovery and /or readiness to change
Collect and maintain data necessary for reporting and compliance activities that provide pertinent reporting as needed
Coordinating with the authorizing and health care or social service entities as necessary to maintain and support the patient's goals.
Facilitate successful transition of care from community partners to inpatient services and MAT outpatient settings based on the level of care needed
Coordinate scheduling of Zepf patients to OB/GYN services with ProMedica
Document patient progress and recommendations in the EHR; all in person and telephonic encounters with also be documented
These services shall be provided in either a community or home setting
Document and bill for all program as allowed according to the agency’s established deadlines
Perform other duties as assigned
Be willing to travel the Northwest Ohio region to help client gain access to care
Help with facilitation of groups for pregnant or parenting individuals

Ability to write clear, concise, accurate notes, data collection, outcome reporting and compliance with laws and standards of confidentiality
Must have the necessary skills and abilities required to comfortably and effectively working with patients, and be responsive to their mental and physical needs
Ability to maintain licensure/credentials

Requirements:
Position Qualifications:
LPN or RN required, bachelor’s degree in nursing, social work, community health, or a related field preferred
3-5 years of related experience required
Chemical Dependency licensure required
Must have a valid driver's license and insurance.
Preferred knowledge of OMHAS, DEA and CARF standards
CPR and First Aid trained, and Crisis Prevention (CPI) trained
Those eligible to drive company vehicles must have a valid driver's license and be eligible for coverage as defined by the agency commercial insurance carrier. Those who drive personal vehicles in the course of business must be able to provide proof of insurance.
