Hospice Nurse Case Manager

Job Overview: We are seeking a compassionate and dedicated Hospice Nurse to join our team. The Hospice Nurse will provide end-of-life care and support to patients and their families during a challenging time. This role requires a high level of empathy, medical knowledge, and professionalism. Responsibilities: Conduct initial and ongoing assessments of patients' physical, emotional, and spiritual needs Develop and implement individualized care plans in collaboration with the interdisciplinary team Administer medications and treatments as prescribed by the physician Provide symptom management and pain control to ensure patient comfort Educate patients and families about the hospice philosophy and end-of-life care Provide emotional support and counseling to patients and their families Coordinate care with other members of the interdisciplinary team, including physicians, social workers, chaplains, and home health aides Document patient care services and progress in an accurate and timely manner Participate in on-call rotation as required Experience: - Previous experience in hospice or palliative care is preferred - Proficiency in medical terminology and medical records management - Strong patient care skills with a focus on comfort and dignity - Knowledge of anatomy and physiology - Understanding of HIPAA regulations - Current nursing license (RN) Qualifications: Current Registered Nurse (RN) license in the state of [MA] Minimum of 1 year of nursing experience, preferably in hospice or palliative care Strong clinical assessment and critical thinking skills Excellent communication and interpersonal skills Compassionate and empathetic nature Ability to work independently and as part of a team Valid driver's license and reliable transportation Preferred Qualifications: Certification in Hospice and Palliative Care Nursing (CHPN) Bachelor of Science in Nursing (BSN) degree Joining our team as a Hospice Nurse offers the opportunity to make a meaningful impact on the lives of patients and their families during a critical time.
