Job Title: RN Case Manager - HospiceLocation: Atlanta, GA Metro Area (Northwest and Northeast of the city: Roswell, Marietta, Alpharetta, Canton, Johns Creek, Kennesaw, etc)Position Type: Full-TimePosition OverviewWe are seeking a compassionate and experienced RN Case Manager to join our hospice team. In this role, you will manage a caseload of patients receiving end-of-life care, ensuring comfort, dignity, and quality of life through individualized, patient-centered care plans.The ideal candidate is a strong clinician with hospice experience who is comfortable supporting patients and families through the end-of-life process and collaborating closely with an interdisciplinary care team.Key ResponsibilitiesCase Management & Care PlanningManage a defined caseload of hospice patients across home and/or facility settingsConduct comprehensive assessments and ongoing reassessmentsDevelop and implement individualized hospice care plans in collaboration with the interdisciplinary teamCoordinate all aspects of patient care to ensure continuity and qualityClinical CareProvide skilled nursing care, including pain and symptom managementAdminister medications and treatments as orderedMonitor patient condition and respond to changes appropriatelyEnsure care aligns with hospice philosophy and physician ordersPatient & Family SupportServe as primary point of contact for patients and familiesProvide education and emotional support related to disease progression and end-of-life careGuide families through the hospice process and care decisionsParticipate in on-call rotation as requiredInterdisciplinary CollaborationWork closely with physicians, social workers, chaplains, and hospice aidesParticipate in interdisciplinary team (IDT) meetingsCommunicate changes in patient status and care needsDocumentation & ComplianceMaintain accurate, timely, and compliant clinical documentationEnsure adherence to all Medicare hospice Conditions of Participation and regulatory requirementsParticipate in audits, surveys, and quality improvement initiativesQualificationsActive Registered Nurse (RN) license in Georgia1-2+ years of nursing experience required; hospice or home health experience strongly preferredStrong clinical assessment and critical thinking skillsExcellent communication and interpersonal abilitiesCompassionate, patient-centered approach to careAbility to work independently and manage a caseload effectivelyCompensation & BenefitsCompetitive salary based on experienceMileage reimbursementHealth, dental, and vision insurancePaid time off and holidaysSupportive team environmentWhat Success Looks LikeEffective pain and symptom management for all patientsHigh patient and family satisfactionTimely and accurate documentationStrong collaboration with interdisciplinary teamApply TodayIf you are a compassionate RN committed to providing high-quality end-of-life care, we encourage you to apply and make a meaningful difference in the lives of patients and families.
