VIRTUAL SOCIAL WORKER-CASE MANAGER

REMOTE Bilingual Social Workers Job Summary: Mystis Adult and Family Services is seeking compassionate AND highly organized Bilingual Social Worker-Case Manager's to provide client-centered services to individuals and families in need. The ideal candidates will be fluent in English e.g., Spanish, Tagalog, etc. and have experience working with diverse populations. This role involves coordinating care, providing referrals to community resources, and ensuring that clients receive the support they need to achieve stability and long-term success. The Bilingual Case Manager will also be responsible for maintaining accurate documentation and working within trauma-informed care and housing-focused frameworks. Key Responsibilities: Client Assessment and Intake: Conduct thorough assessments to identify clients’ needs, strengths, and barriers to housing, healthcare, employment, and other essential services. Develop Care Plans: Work with clients to develop individualized care plans based on their unique circumstances, ensuring plans are actionable and achievable. Referral and Resource Coordination: Connect clients with appropriate external and internal resources, including housing, mental health services, employment programs, medical care, and financial assistance. Client Advocacy: Serve as an advocate for clients by helping them navigate systems such as healthcare, housing, and social services. Communicate and coordinate with external service providers to ensure seamless client support. Case Documentation and Reporting: Maintain accurate and timely records of client interactions, assessments, and progress toward goals in accordance with organizational and legal standards. Follow-Up and Monitoring: Conduct regular follow-ups to ensure clients are successfully accessing services, meeting their goals, and adjusting care plans as needed. Bilingual Support: Provide culturally competent services to non-English-speaking clients by conducting all case management activities, including assessments and follow-ups, in the client’s preferred language. Collaboration: Work with internal teams, including supervisors and other case managers, to ensure the highest level of service delivery. Attend case conferences and team meetings as required. Crisis Intervention: Provide immediate support and intervention for clients in crisis, connecting them with urgent services or emergency housing as needed. Compliance and Confidentiality: Ensure all client interactions and documentation comply with organizational policies and state/federal regulations. Maintain strict confidentiality of all client information. Qualifications: Bilingual Required: Fluent in English and Spanish. 2+ years of experience in case management, social services, or a related field. Experience working with vulnerable populations, including those experiencing homelessness, mental health issues, substance use, or domestic violence. Familiarity with community resources and local social services, particularly those related to housing, healthcare, and employment. Strong organizational skills, with the ability to manage a caseload and meet deadlines in a fast-paced environment. Excellent communication skills (verbal and written) in both English and the preferred second language. Problem-solving and critical thinking: Ability to handle complex client situations and provide appropriate support. Cultural competence: Ability to provide trauma-informed and culturally sensitive care to clients from diverse backgrounds. Proficiency in using case management software and general computer literacy (e.g., Microsoft Office Suite, Google Workspace). Ability to work both independently and collaboratively in a remote or in-person setting. Preferred Qualifications: Experience working in housing services, mental health care, or social work environments. Knowledge of trauma-informed care practices. Experience in navigating Medicaid, Medicare, or other public assistance programs. Work Environment: Remote, hybrid, AND in-person depending on the client’s needs and company requirements. Regular use of phone, email, and video conferencing tools to communicate with clients and teams. Must be able to handle emotionally challenging or complex cases with empathy and professionalism.
