Job DetailsDescriptionLicensed Practical Nurse (LPN): Adult Residential ServicesSalary Range: $30.00 - $33.00 /hourAbout DDIWith nearly 65 years of service, DDI provides a full continuum of care -- from early intervention to residential, day habilitation, and vocational programs -- for over 1,600 children and adults with autism and other developmental disabilities. DDI is driven by a dedicated staff and a belief in every individual's potential.The Role: The LPN supports DDI's residential programs and serves as a medical liaison for IDT team. The LPN updates the written plan for each person and maintains records for each individuals as well as appropriate in the planning and implementation of plan with DDI personnel. NY State Licensed Practical Nurse required.What You'll DoParticipates in IDT conferences, as required.Develops written plan for each person with RN Supervisor. Modifies nursing plan as necessary.Maintains medical records for each individual.Participates as appropriate in planning and implementation of training of DDI personnel.Accompanies individuals to medical appointments as assigned.Ensure proper medical follow-up through on call system.Ensures all medications are administered and stored as necessary.Attends monthly Nursing Meetings.Completes appropriate incident reporting paperwork as required.Ensures compliance with OPWDD regulatory standards.Liaison with family members regarding medical issues.Preparation of monthly Medication Administration Records.24-hour on call for medical emergencies with rotating weekend coverage required.What You Need For The RoleN.Y. State Licensed Practical Nurse License requiredN.Y.
