DescriptionLCSW Specialty - Center for Healthy Aging - Rhinebeck, NYThe LCSW provides integrated behavioral health services within the primary care setting, delivering assessments, brief therapeutic interventions, and care coordination for adolescent and adult patients. This role supports improved psychosocial functioning,facilitatesaccess toappropriate levelsof care, and collaborates closely with providers and the multidisciplinary team to ensure comprehensive,patient centeredcare.Key ResponsibilitiesConduct behavioral health assessments and develop individualized treatmentand transition plans.Provideevidence-basedinterventions (CBT, DBT, MI, SBIRT, mindfulness,trauma‑informedcare).Manage acute behavioral health concerns, including crisis intervention and suicide risk evaluation.Support behavioral health integration in primary care through collaboration, consultation, and care coordination.Facilitate referrals to higher levels of care and connect patients to community and hospital resources.Document all care accurately andtimelywhile adhering to regulatory and compliance standards.Participate in quality initiatives,maintainprofessional knowledge, and modelNorthwellHealth values.QualificationsLicensed Clinical Social Worker (LCSW) in CT or NY, depending on work location.Minimum 3–5 years of clinical experience.Experience in medical social work, crisis intervention, behavioral health, or integrated care preferred.Strong clinical judgment, diagnostic skills, communication abilities, andproficiencywithevidence-basedmodalities.SkillsExpertiseindiagnostic assessment, treatment planning, and crisis management.Knowledge of mental health and substance use conditions, includingcooccurringdisorders.Strong communication, documentation, and organizational skills.Ability to collaborate effectively with medical providers, care teams, and community partners.For more information, please reach out to stephanie.ramirez@nuvancehealth.orgCompany: Nuvance Health Med Practice PCOrg Unit: 1136Department: Center For Healthy AgingExempt: YesSalary Range: $33.21 - $61.68 Hourly
