Social Worker

E&V Adult Day Health Care Center – Compton, CA E&V Adult Day Health Care Center is seeking a compassionate, organized, and dedicated Social Worker to join our multidisciplinary team. We are looking for someone who is passionate about supporting older adults and helping participants maintain their independence and quality of life. Schedule: Monday – Friday 8:00 AM – 2:00 PM Part-Time Preferred Qualifications: Bilingual in Spanish and English (strong verbal and written communication skills preferred). Responsibilities: Conduct psychosocial assessments and ongoing participant evaluations. Monitor participants' emotional, social, and behavioral well-being. Complete home assessments as needed to evaluate living conditions and identify participant needs. Develop individualized care plans and provide ongoing case management. Facilitate problem-solving and support discussion groups. Assist participants and families with accessing community resources, benefits, and supportive services. Provide counseling, education, and emotional support to participants and caregivers. Serve as a liaison between participants, families, caregivers, healthcare providers, and community agencies. Participate in Multidisciplinary Team (MDT) meetings and contribute to participant care planning. Maintain timely, accurate, and confidential documentation of all assessments, interventions, referrals, and participant interactions. Collaborate with nursing and rehabilitation staff to ensure comprehensive participant care. Minimum Qualifications (California CBAS Requirements): Applicants must meet one of the following qualifications: Bachelor's degree in Social Work from an accredited school and at least two (2) years of experience providing social services in aging, healthcare, or long-term care; or Master's degree in Social Work from an accredited school of social work; or Master's degree in Psychology, Gerontology, or Counseling from an accredited institution and at least one (1) year of experience providing social services in aging, healthcare, or long-term care.
