Schedule Friday - Monday PM | 2:30 PM -10:45 PM Pay Range $18-$19 What You’ll Do Assist residents with Activities of Daily Living (ADLs) including, but not limited to continence care, showering, grooming, dressing, escorts, transfer assistance, repositioning, and meal assistance. Assist residents in remaining clean, dignified, and presentable. During your shift, utilize a company‑provided phone to access care plans, make emergency phone calls, and chart care accordingly. Follow a care plan and document provided care accordingly. Observe, document, and report to the Care Manager Team any physical, emotional, or mental changes, as well as unusual resident behaviors (e.g., speech, feeding patterns, toileting), to support assessment and care planning. Read end‑of‑shift notes from previous shifts and attend daily change‑of‑shift meetings. Embrace teamwork and collaboration among all staff. Train New Hires Be the point person for all Care Partners for general workplace knowledge. Conduct weekly check‑ins at work for New Hires. Assist the Care Manager by providing direction on job specific experiences and expectations May be the point person for emergencies, residents, and family members, should a manager or supervisor not be present Keep residents’ rooms in proper order throughout the day, including tidying rooms, making beds, and putting clothes away. Assist residents by providing for their comfort, safety & enjoyment. Follow proper incident procedures for all incident types (i.e. elopement, suicide ideations, falls, skin breakdown, resident altercations, etc.) Promotes appropriate levels of independence in resident care and demonstrates awareness of safety for both and self in providing care. Utilize durable medical equipment per training. Communicate resident supply levels to the supervisor. Respond to fall alerts, bed alarms, emergency pendants, and call lights in a timely manner. This may include being captured on video camera. Assist with resident meals, including serving, cleaning, and setting up the dining area. Record resident meal consumption and report changes to the manager. Follow infection control procedures according to training. Practice Positive Approach to Care (PAC) toward residents. Attend and remain up to date on all company‑provided training, and attend periodic staff meetings, in‑service training, and mandatory meetings as required. Observe Residents’ Rights, as posted within the facility. Mandated reporting – if you suspect any kind of verbal, physical, emotional, financial, or neglectful treatment toward a resident or staff member, report it to your supervisor immediately. In Emergencies, assist residents according to the Emergency Plan of the facility. Qualifications Must be at least eighteen (18) years or older, per Title 22 requirements Prior caregiving experience preferred but not required – we will train the right candidate! #J-18808-Ljbffr
