This role oversees day-to-day workflow, ensures regulatory compliance, supports quality outcomes, and serves as liaison between multidisciplinary teams to facilitate safe, patient-centered transplant care.POSITION EDUCATION/ QUALIFICATIONS:Graduation from an accredited school of nursing as a Registered NurseBachelor's degree in nursing required.Current RN licensure in the State of Texas or Compact state required.Current American Heart Association Basic Life Support (BLS) required.Advanced Cardiovascular Life Support (ACLS) Healthcare Professional certification preferred.Bilingual- English/Spanish preferred.JOB KNOWLEDGE/EXPERIENCE:• Minimum of three (3) years of experience as transplant coordinator required.• Certified Clinical Transplant Coordinator (CCTC) preferred.• Prior leadership or charge nurse role experience preferred.• Strong leadership and team-building skills• Effective communication skills• Ability to mentor, lead, and develop team members for improved functioning and effectiveness.• Ability to manage multiple priorities in a fast-paced environment.• In-depth knowledge of transplant regulations (UNOS/OPTN, CMS)Responsibilities:POSITION RESPONSIBILITIES:• Manage a caseload of patients with end-stage organ disease and/or living donors in various stages the transplant process and in varying levels of complexity. Participate in the multidisciplinary patient care team, collaborating with physicians, surgeons, advanced practice providers, social workers, pharmacists, financial coordinators, nursing personnel, and other health disciplines to coordinate the entire transplant process. Provides education to transplant candidates, recipients, caregivers, and living donors. Provides continuity of care between inpatient admissions and outpatient visits.• Supervise and support transplant coordinators to ensure regulatory compliance, efficiency, and high-quality patient-centered care.• Provide onboarding, training, mentoring, and performance feedback to transplant coordinators.• Assist Transplant Director with staffing schedules, workload distribution, and coverage planning.• Serve as clinical resource and escalation point for transplant coordinators.• Collaborates with all parties involved to provide the most cost-effective and appropriate care to patients.• Systematically ensures that all plans of care are congruent with patients' needs and align with best practices.• Ensures that the quality of the nursing care provided demonstrates compassion and a strong awareness of resource optimization.• Maintain current knowledge of UNOS/OPTN policies.• Ensure adherence to UNOS/OPTN, CMS, regulatory, and institutional policies.• Maintain accurate and timely documentation in EMR and transplant systems.• Support audits, site surveys, and regulatory reporting requirements.• Participate in quality improvement and patient safety efforts.• Identify workflow gaps and implement process improvements to promote efficiency and effectiveness.• Assist leadership with policy development and implementation.• Provide in-service for nursing staff and presentations for any outside organization or groups.• Participate in outreach and education activities as needed.• Participate in quality monitoring and process improvement activities and programs.• Performs on-call, transplant coordinator, and other duties as assigned.LINES OF REPSONSIBILITIES:(Chain-of-command) Transplant Director → 2. Working Conditions: The individual spends over 95% of his/her time in an air-conditioned environment with varying exposures to noise.
