Position Overview: Specialty:Family PracticePost-Acute Care PracticeStart Date:ASAPContract Length:OngoingPosition Type:Locum TenensCoverage Type:Scheduled Clinical Hours Only Key Responsibilities: Patient Care:Provide high-quality, safe, and cost-effective care to patients in post-acute care communities.Manage a caseload of 15-20 patients per day, focusing on the unique needs of each individual.Collaboration:Work closely with interdisciplinary teams, including hospice, palliative care, and home care agencies, to ensure safe transitions of care.Collaborate with other clinicians to follow CMS guidelines and maintain high standards of care.Documentation and Billing:Ensure timely documentation and billing through our EMR system (Gehrimed), meeting Medicare guidelines and ensuring compliance with all relevant regulations. Qualifications: Experience:Willing to train new candidates; experienced candidates may be eligible for increased compensation due to prior experience in a post-acute care setting.Board-certified or board-eligible in Family Medicine or a related specialty.
