As a recipient of funding from the Health Resources and Services Administration (HRSA), CAMcare is committed to eliminating health disparities and improving health outcomes by delivering comprehensive care regardless of ability to pay.Position Summary:The Float Registered Nurse (RN) plays a vital role in delivering high-quality, patient-centered care across multiple CAMcare sites, including health centers and affiliated senior living facilities. This position requires adaptability, clinical expertise, and a collaborative spirit to ensure continuity of services during staffing shortages, PTO coverage, or surges in patient volume.Essential Duties and Responsibilities:Clinical Care - Health CentersDeliver direct patient care in accordance with CAMcare's protocols and evidence-based guidelines.Assist providers during exams and procedures; offer culturally sensitive patient education.Administer immunizations, medications, and injections safely and accurately.Perform triage assessments and prioritize patient care in high-volume environments.Maintain accurate and timely documentation in the Electronic Health Record (EHR).Support infection control, chronic disease management, and quality improvement initiatives.Senior Living Facility DutiesConduct scheduled wellness visits and as-needed assessments at designated senior living facilities.Monitor and manage residents' chronic and acute medical needs.Perform medication reconciliation and provide health coaching and education to residents and families.Collaborate with CAMcare providers and facility staff to coordinate care plans.Communicate clinical updates and recommendations to families and caregivers.Float ResponsibilitiesProvide clinical coverage across CAMcare's Primary Care, Pediatrics, and Women's Health departments.Monitor clinical pools to assist in completion of prior authorizations, prescription requests and clinical data entry as assignedFill in for RNs who are on leave or during periods of increased patient demand.Quickly adapt to varying site-specific workflows and team dynamics.Serve as a reliable resource in maintaining operational stability and service excellence.Minimum Qualifications:Current Registered Nurse (RN) license in the State of New Jersey.CPR/BLS certification required; ACLS preferred.Minimum of 2 years' clinical nursing experience preferred, ideally in ambulatory or FQHC settings.Experience working with geriatric populations and electronic health records (EHRs) preferred.Valid driver's license and reliable transportation to travel between sites.
