Job Summary:
Our Award-Winning Client, a nationally recognized leader in healthcare that has won numerous awards, is seeking a Licensed Clinical Social Worker (LCSW) for full-time night shifts. The role involves conducting psychosocial assessments, developing plans to address the needs of patients with complex social issues, and coordinating post-discharge services as needed.
Key Responsibilities:
Assist with adoptions, and cases of abuse and neglect, including making referrals to appropriate state and federal agencies.
Engage in weekly Complex Case Review meetings and daily patient care conferences.
Provide resources and referrals to community agencies for ongoing counseling and continued care.
Maintain an updated list of post-discharge care providers.
Communicate effectively with post-discharge care providers.
Update Case Managers on any variances to post-discharge plans.
Prepare and present reports to the Director of Case Management and hospital leadership as requested.
Stay informed about regulatory requirements and community resources.
Perform other duties as assigned.
Additional Responsibilities: Working under the guidance of the Social Work Manager/LCSW, the social worker will deliver age-appropriate social work services to patients and their caregivers, in line with hospital policies, procedures, and regulatory standards. As an integral member of the healthcare team, the Social Worker II will conduct assessments, coordinate treatment planning, provide information and referrals to community resources, and offer other social work services to address the complex needs of patients and families in both hospital and clinic settings. The MSW will seek supervision from a Licensed Clinical Social Worker as needed and will use the Patient Medical Record and HPF as primary information systems.
Candidate Profile: Ideal candidates should have less than one year of experience, hold a Master's degree, and not require management experience. This is an entry-level position, and candidates should have no willingness to travel.
