In this role, you will make a difference in the lives of seniors and people with Dementia, Alzheimer's, Parkinsons, stroke, post-surgery, swallowing difficulty, and more by enhancing their quality of life. You will be working with Ese, the RN, other care coordinators who will be designing the care plan with the families.
