Travel Physical Therapist - Home Health

American Traveler is seeking a licensed Physical Therapist (PT) for a Home Health assignment requiring at least 1 year of PT experience and a valid driver's license for patient home visits within an assigned territory.Job Details• Home health setting - clinician travels to patients' homes within an assigned geographic territory,• Caseload is managed within the assigned territory or surrounding areas to fill an 8-hour day,• Schedule is Monday-Friday, 8am-5pm, with no on-call requirements,• Homecare Homebase EMR experience is preferred,• Cerner is used for charting,Job Requirements• Current SC PT license or compact license required; license required if applicable,• Current AHA BLS certification required,• Minimum 1 year of experience as a Physical Therapist preferred; home health experience strongly preferred,• Graduate of an accredited Physical Therapy program required,• Valid, current driver's license required - this position requires operating a vehicle,• Proficiency in English (speaking, reading, and writing) required,• Demonstrated knowledge of therapeutic tasks, rehabilitative activities, and orthotic/prosthetic/assistive devices,• Strong organizational and time management skills required,• 2 professional references required for consideration,Additional Information• Primary responsibilities include evaluating, planning, and delivering patient care to address deficits in self-care, mobility, safety, range of motion, and strength,• Responsible for supervising and directing care provided by PTAs and Home Health Aides to ensure treatment goals are met,• Documents all care, coordination of services, orders, and goals in the patient's EMR accurately and in a timely manner,• Coordinates with other disciplines, agencies, families, and caregivers to meet patient care needs,• Candidates must live more than 50 miles from the facility to be eligible; local candidates are not accepted,• Travelers may float to a nearby sister facility,
