Position Overview
The Care Connection Program Manager is responsible for the management, expansion, and day-to-day operation of Care Connection Manatee, a navigation program designed to assist uninsured individuals in Manatee County with accessing medical care and support services. This role serves as an advocate for uninsured patients, connects them to appropriate healthcare resources, and coordinates with community partners, providers, and internal staff. The position includes program enrollment oversight, reporting support, and inter-agency coordination.
Success in This Role
· Continued expansion and maturation of the Care Connection Manatee program
· Effective implementation of outreach, marketing, and community education efforts
· Successful navigation and enrollment of uninsured clients into appropriate care and assistance programs
· Strong coordination between clients, clinics, partner organizations, and internal staff
Primary Responsibilities:
Client Navigation & Advocacy
· Conduct one-on-one meetings and phone calls with clients to assess needs and assist them in accessing medical care
· Advocate for uninsured clients to ensure successful connection to healthcare resources
· Assist clients with applications for government and private assistance programs
· Think creatively and proactively to resolve barriers to care for uninsured patients
Program Enrollment & Compliance
· Oversee program enrollment, including verification of eligibility requirements
· Ensure compliance with program guidelines and accurate collection of all required documentation
· Ensure accurate data entry and correction of client records and communication notes
Program Management & Operations
· Manage the daily operations of Care Connection Manatee
· Monitor program performance and client outcomes
· Assist the Operations Manager with report preparation and data review
· Support development of more efficient logistical workflows to better serve clients
Community Outreach & Partnerships
· Serve as liaison between clients, clinics, outside organizations, and partners
· Ensure updates and changes between all parties are clearly communicated and understood
· Provide on-site assistance at community partner locations, including regular coordination at partner offices
· Advocate for Primary Care Physician and specialized provider enrollment in the Pro Bono Program
Outreach, Education & Representation
· Present to local groups, providers, and potential partners
· Assist with outreach, marketing, and communication efforts for the Care Connection program
· Represent We Care Manatee at community meetings and outreach events
Qualifications & Skills
· Fluent in Spanish (required)
· Knowledge of local, state, and federal health coverage programs
· Bachelor’s degree in social work, public health, healthcare, or related field preferred
Will consider 3 – 5 years of comparable work experience
· Strong advocacy skills and ability to think creatively for uninsured clients
· Self-motivated, assertive and pro-active with the ability to solve problems independently
· Strong interpersonal, written, and verbal communication skills
· Proficient computer skills to work in the office, remotely, and at partner locations

Pay: $60,000.00 - $65,000.00 per year
