Each year, the organization provides Interim and Immediate Housing to approximately 150 homeless families in the Kansas City area, empowering them to break the cycle of homelessness. Through advocacy, financial education, and life-skills training, residents achieve debt reduction, savings growth, and better self-sufficiency. Community LINC also offers mental wellness services, academic support for children, and Aftercare Services to ensure families maintain independence after securing permanent housing. Dedicated staff and volunteers work together to make a lasting impact on the community.Job Title: Outreach NavigatorLocation: Community LINC, 4012 Troost Ave KCMO 64110Job Summary: We are seeking an enthusiastic, self-motivated, results-oriented individual to serve as a primary point of contact for persons experiencing homelessness seeking services through the Coordinated Entry System (CES) which provides a streamlined ease-of-access to homeless services. This person will have the sole responsibility of providing day-to-day support to persons on the By-Name Registry List (BNL) which serves as a waiting list for those seeking permanent housing. Key Responsibilities:Program Management· Provide By-Name Registry list status updates upon request.· Assist with obtaining crucial documents prior to housing placement i.e. social security cards. · Maintain contact with clients to the greatest degree possible. · Assist clients with obtaining needed resources. · Assist clients with navigating the homeless services system. · Conduct Vulnerability Assessments upon request. · Work closely with partnering agencies. · Actively network and research resources and connect clients to those resources to help them stabilize. · Complete data entry into mandated HMIS system and internal data systems · Work closely with other program staff to ensure supportive services are in place for clients. · Provide support to ensure clients are transitioned into stability· Teach life skills class to campus families once a month or as neededRecord Keeping/Reporting· Develop housing stability plans and update progression with clients· Document client contact and activities in HMIS as prescribed · Produce/create other reports as requested. Administrative/Other Duties· Attend team meetings and others as requested· Attend and support resident activities and events as assigned· Participate in COC monthly meetingsQualifications:Understand the complexities of working with the homeless population. Be able to adapt to an ever-changing environment. Have exceptional self-management and organizational skills. Perform simultaneous tasks. Exhibit professionalism always. Work well within a team. Exhibit strong verbal and written communication skills. Must be able to travel often in the KC Metro.
