Prepares initial diagnoses and completes reports on individuals. Prepares authorizations for treatment and services on clients. Monitors client activity to assess need for continued treatment. Makes referrals for clients to appropriate treatment facilities and community agencies as needed. Arranges psychiatric evaluations for clients and follows up on medical checks as needed. Provides ongoing case coordination and support with treatment-related needs, as identified. Prepares progress notes and charts case management sessions. Completes required paperwork and reports. Participates in interagency activities designed to attain common goals for clients. Participates in each provider's staff meeting to discuss County clients.Other Duties Provides community outreach services as needed. Attends meetings and/or training as necessary. Receives and works to resolve program complaints. Performs other job-related duties as required.
