The PMHNP works closely with therapists and support staff to ensure comprehensive, patient-centered care.Key ResponsibilitiesConduct comprehensive psychiatric evaluations and mental health assessmentsDiagnose psychiatric and behavioral health conditions across the lifespanDevelop individualized treatment plans using evidence-based practicesPrescribe, monitor, and manage psychiatric medications in accordance with New York State regulationsMonitor treatment effectiveness and adjust interventions as clinically appropriateMaintain timely and accurate clinical documentation in the electronic health recordCollaborate with interdisciplinary team members to coordinate integrated care services and support each other to ensure patients are covered with medication needs.Provide patient and family education regarding diagnosis, medications, and treatment recommendationsParticipate in quality improvement and compliance initiatives consistent with FQHC standardsQualificationsActive New York State Nurse Practitioner license with prescriptive authorityMaster's degree in Psychiatric Mental Health Nursing from an accredited programBoard Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP)Active DEA registrationMinimum of one (1) year of Psychiatric Nurse Practitioner experience required; three (3) years preferredExperience in outpatient, community mental health, FQHC, or integrated care settings preferredStrong interpersonal, communication, and collaborative care skillsAbout Our Behavioral Health ProgramOur behavioral health team works within an integrated healthcare model focused on whole-person care.
