The individual must demonstrate knowledge of the principles of growth and development throughout the lifespan. Possess the ability to assess data that reflects the patient's status and interpret the information required to identify each patient's needs. Provide direct and indirect skilled nursing care for assigned residents. Follow defined policies and procedures under the supervision of a Registered Nurse. Maintain a collaborative relationship with other nursing staff, physicians, and ancillary staff. Maintain an appropriate level of knowledge and clinical expertise through continuing education and in-service programs. The LPN should be able to provide care to geriatric residents. Collect data relevant to the resident's physical and psychosocial condition on an ongoing basis. Communicate findings to the Registered Nurse and other members of the Interdisciplinary Team. Contribute to the creation of a developmentally age-appropriate and culturally relevant plan of care. Reflect nursing diagnoses and interventions to achieve desired outcomes involving the resident, family, and healthcare team. Promote a healthy and safe environment. Recommend revisions to the plan of care after evaluating resident outcomes. Indicate the need for referrals to ensure all aspects of the resident's care have been addressed. Collaborate with the resident, family, and members of the Interdisciplinary Team to assure satisfaction. Maintain current knowledge and competency in Nursing Practice. Participate in mentoring and orientation of staff. Consider factors related to safety, cost, and impact in planning and delivery of care. Under the direction of the Registered Nurse, provide leadership in the professional practice setting. Demonstrate knowledge and competency. Occupational exposure to bloodborne pathogens is reasonably anticipated.
