Social Worker II - Lead

Our Client is seeking a Lead Social Worker for our to ensure that we continue to provide all patients with high quality, efficient care. Hours: 8:00-4:30 pm (rotating weekends)Job DutiesPlans, organizes, directs, defines and coordinates clinical services necessary to meet the legal, organizational, medical staff and regulatory agencies guidelines.Provide case management, education and discharge planning. Performs community and educational activities.Supports and coordinates the social service function for patients and their families, providing discharge planning pertaining to illness, hospitalization and financial concerns and facilitates post discharge planningWorks within the structure to negotiate and resolve problems and conflicts during the patient stay to ensure continuity of care.QualificationsMaster's degree in Social Work (MSW) requiredFlorida license required (LCSW - Licensed Clinical Social Worker)Current BLS (Basic Life Support) required (within 30 days of employment start date)Current CPI (Crisis Prevention Intervention) required (within 30 days of employment start date)Minimum of two (2) years of experience in Social Services and/or Case Management experience preferredExperience with Behavioral Health patients a plusSuccessful Leadership / Management experience preferredBenefitsMedicalDentalVisionEmployee Stock Purchase PlanPaid Time OffEducation Assistance401(k) Plan with 100% matchand many more to be discussed when you apply.
