This patient-centered role focuses on providing comprehensive primary care to patients ages 2 and older, including children, adults, and families, with a special emphasis on cardiology collaboration. Key Responsibilities: Provide comprehensive, evidence-based primary care to patients ages 2 and older, including children, adults, and families, in an outpatient setting. Diagnose, treat, and manage acute and chronic medical conditions, ensuring continuity of care and appropriate follow-up. Perform preventive care services, including annual physicals, health screenings, immunizations, and wellness counseling. Deliver basic women's health services, including routine gynecologic care, contraceptive counseling, and sexual wellness evaluations. Address sexual health and wellness concerns with a patient-centered, confidential, and culturally sensitive approach. Develop long-term therapeutic relationships with patients and families to promote overall health and disease prevention. Collaborate closely with in-house specialists and clinical staff to ensure coordinated, high-quality patient care. Utilize EHR systems efficiently with the support of human scribes and AI scribe technology to maintain accurate clinical documentation. Educate patients and families on diagnoses, treatment plans, medications, and lifestyle modifications.
