MT - Home Hospice RN, Days M-F with Call

Job Summary: TheRN - Case Manager provides skilled nursing, care, and case management forassigned patients in their place of residence; admits patients onto services;coordinates and integrates services to ensure continuity of care; facilitatesthe achievement of quality, clinical and cost outcomes; negotiates, procuresand coordinates services and resources needed by the patient or family; createsopportunities and systems to enhance outcomes; and uses the nursing process ofassessment, planning and implementation of care, including patient/familyinstruction and evaluation through an inter-disciplinary team approach. Thisposition may start as a Visiting RN and move to Case Manager. The same clinicalresponsibilities apply to both designations. To move to Case Manager, theincumbent must carry a half case load as described in the Collective BargainingAgreement with the Montana Nurses Association Local #32. Essential Duties andResponsibilities: 1. Provides skilled nursing and coordinatescare to patients and family members in a residential setting through a teamapproach and based upon systematic evaluation as authorized by a physician. 2. Observes, assesses, evaluates, and plansinterventions in relation to patients' physical and psychosocial statuses,including changes in condition and/or reactions to treatments or medicationsfor the duration of contact with patients. 3. Oversees care provided by Home Health Aidesand CNAs to assigned patients and evaluates compliance with plan of care. 4. Assures continuity of care through effectivecommunication, information exchange and case conferences with otherdisciplines, community health, support and social agencies. 5. Provides quality care and appropriatesymptom management for positive clinical outcomes and promotes optimal healthand quality of life through teaching, counseling, appropriate interventions andreferrals. 6. Maintains accurate, current and completepatient records in accordance with Agency standards and OASIS requirements aswell as knowledge of current reimbursement rules and regulations. 7. Participates in team scheduling coordinationand "on-call" rotation schedule as necessary to meet patient care needs byassessing and evaluating patient needs via telephone and advising or makinghome visits as required. 8. Participates in quality improvementactivities and attends in-service training as assigned. 9. Performs other duties as assigned bysupervisor. Minimum Qualifications: Qualificationsinclude a current RN license in the State of Montana (BSN preferred), a validdriver's license, appropriate auto insurance, and reliabletransportation. 2 years of recent acute care, home care, or hospiceexperience including such clinical skills as venipuncture, catheterization,wound care and/or parenteral therapy preferred.
