We offer a pleasant, family-friendly environment and a wonderful reputation providing services to our residents, families and local community.TEAMWORK EXCELLENT BENEFITS STABLE WORK ENVIRONMENTDaily Pay option available!!Duties and Responsibilities:Maintaining and/or improving resident's ability to control everyday physical needs and mental/psychosocial needsIdentifying individual social and emotional needsDeveloping and maintaining individualized care plansParticipating in the planning of the resident's admission, community re-entry or transfer to another facilityParticipating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the residentWork with the finance team to ensure resident is educated and the proper documents are in place for the resident during their stayWork with the resident on discharge and get necessary DME or home care in place so that the resident is safe when they return to the communityDemonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning social services programs.Demonstrates knowledge, skills, and techniques necessary to plan social services programs for residents with the following needs: therapeutic, neurological and dementia, dialysis, IV therapy, and infectious diseases if applicable.Promptly follows-up on admission inquiries, evaluates and assesses the needs of prospective residents to determine appropriateness of facility placementQualifications/Requirements:RN, BSW or MSW preferred, or Bachelors degree in related fieldAt least one year experience in Social Work field requiredExperience in long term care and/or sub-acute setting preferredBenefits:Health insuranceDental insuranceVision insuranceRetirement plan/401kPaid time offFlexible schedule#HP24
