People Caring For And About People.Your Role as an MDS Nurse:Conduct initial and ongoing MDS assessments in accordance with federal, state, and facility regulationsAccurately complete all sections of the MDS, including RAI and care planning documentationCollaborate with the interdisciplinary team to ensure accurate documentation of resident needs and goalsProvide education and guidance to staff on MDS requirements and regulatory standardsEnsure compliance with all federal and state regulations related to long-term careParticipate in quality improvement initiatives and regulatory survey processesMaintain accurate, up-to-date records of MDS assessments and care plansCoordinate timely transmission of MDS data to appropriate agenciesParticipate in interdisciplinary care plan meetings and provide clinical inputMonitor and update care plans based on changes in resident conditionMaintain case-mix index at target levelsCommunicate effectively with physicians, staff, residents, families, and vendorsFollow all facility policies, including infection control, safety, and HIPAA regulationsMaintain a calm, supportive, and professional environmentAttend required trainings, in-services, and meetingsWhat We're Looking For:Graduate of an accredited School of NursingCurrent Ohio LPN or RN licenseCurrent CPR certificationMinimum of 1 year of experience as an MDS Nurse required3+ years of experience in long-term care or skilled nursing preferredStrong knowledge of federal and state long-term care regulationsExcellent attention to detail and accuracyStrong organizational and time-management skillsEffective communication and interpersonal skillsAbility to work independently and as part of a teamProficiency in Microsoft Office and electronic medical records (PointClickCare preferred)Physically able to meet job requirements, including lifting and assisting residents as neededYour Next Step Starts Here!This is more than a job-it's an opportunity to make a meaningful difference every day.Caring Place Healthcare Group is an equal opportunity employer.
