Job Description The Medical Social Worker collaborates with the multidisciplinary team to provide input in the development of the plan of care for those patients requiring social work intervention. The Medical Social Worker provides medically necessary intervention to patients who have social or emotional problems that are, or are expected to be, impediment(s) to the effective treatment of the medical condition and/or rate of recovery. Additionally, the Medical Social Worker provides these services in accordance with physician’s orders.Essential Duties And Responsibilities:Provides medically necessary social work intervention in accordance with physician’s ordersAssesses patient’s psychosocial needs; plans for, evaluates, and modifies interventions to meet the established patient goalsCollaborates with other disciplines to plan and evaluate team goals for each patientFacilitates patient referrals to community agencies when appropriateProvides financial counseling when necessary to achieve the treatment goals established by the multidisciplinary teamAssists patient/family/caregivers in planning for present and future psychosocial needs related to the patient’s current status and prognosis, including terminal illnessAdheres to company policies and procedures while providing services and preparing/submitting required paperworkObserves and reports patient progress and/or changes in patient status to physician and appropriate multidisciplinary team membersFunctions as a resource person for other members of the health care team, providing inservice education and consultation in understanding the social and emotional factors related to health problems.Maintains knowledge of the field including Medicare, Medicaid, and other third-party reimbursement sources.Participates in planning and implementing quality improvement activities as necessaryProvides positive experiences in assisting patients, families, physicians, other departmental employees, referral resources, service agencies, and others.Adheres to facility and Maxim policies at all timesMinimum Requirements:Baccalaureate degree in Social Work (BSW) or Master’s degree from a school of social work accredited by the Council on Social Work Education per state/client requirements requiredMinimum of one (1) year of social work experience in a health care environment in the last two years preferredComplies with all relevant professional standards of practice Participation and completion of Amergis' Competency program when applicableCurrent CPR if applicableTB questionnaire, PPD or chest x-ray if applicable Current Health certificate (per contract or state regulation) Must meet all federal, state and local requirements Successful completion of new hire training as applicable to job site Understand patient confidentiality and HIPAA requirements Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required Computer proficiency required Must be at least 18 years of age Company DescriptionWe are a national healthcare staffing company with local roots committed to personal relationships with our clients in recruitment, hiring and placement of a quality and diverse healthcare workforce.Company DescriptionWe are a national healthcare staffing company with local roots committed to personal relationships with our clients in recruitment, hiring and placement of a quality and diverse healthcare workforce.
