Join our compassionate community as a Care Transition Specialist! In this role, you will assess member needs and guide them and their families towards resources that foster optimal well-being and health. As a key member of our team, you will play a crucial role in facilitating smooth transitions for members in and out of various Medicaid programs, ensuring that they receive the best possible care.Key Responsibilities:Support and manage initial and ongoing member transitions within Indiana Medicaid programs and between care settings.Collaborate with the Member Advocate Coordinator and other departments to ensure seamless coordination of care.Ensure the transfer of necessary prior authorization decisions, clinical information, and wellness program details.Assist with transitions from custodial settings to home and community-based environments.Conduct telephonic and in-person meetings with long-term care members, healthcare professionals, and families to prevent unnecessary custodial placements.Assess members' needs to develop tailored care plans and coordinate appropriate services.Guide members and families to suitable resources by facilitating interactions with various stakeholders in their care.Required Qualifications:Bachelor's degree in health or human services field.2+ years of relevant experience.Proficiency in Microsoft Office Suite, with strong attention to detail.Excellent verbal and written communication skills.Passion for improving consumer experiences and outcomes.This position involves patient interaction and requires participation in Humana's TB screening program. A valid driver's license, personal vehicle liability insurance, and a dedicated home office space are also necessary.Preferred Qualifications:Master's Degree.State licensure in relevant field.Case Management Certification (CCM).Experience with Medicare & Medicaid recipients.Familiarity with community health resources.Additional Information:Some travel to Humana offices may be required for training or meetings. This is a full-time position with scheduled hours of 40 per week.Salary:The estimated salary range for this full-time position starts at $59,300 and goes up to $80,900 per year, based on skills, experience, and other relevant factors.
