Direct Support Professional/Caregiver

Center for Human Development (CHD) Meadows Homes provides residential housing supports for individuals with intellectual disabilities. While supporting and encouraging them to live full active lives. Direct Care professionals provide a supportive residential environment for developmentally disabled adults living in a community-based group home. They seek to provide an environment that encourages growth and supports individuals to live as independently as possible in all aspects of life.Responsibilities Supporting clients' activities of daily living.Personal careBathing, toileting, grooming, teeth brushing, hair careHousehold choresDishes, laundry, shopping, preparing meals based on dining guidelinesTransportation Advocating for individuals' within various community settings,Maintaining social relationshipsVisiting with friends and familyAssisting individuals to make new friendsOutingsBowling, dances, vacations, religious events Support individuals with medical needsVital signsMedical recordsMedical appointmentsMedication administration (MAP certification)Medical devicesHoyer lift, wheelchair, ostomy, walker, shower chairCaring for individuals with complex diagnosesAutism, dementia, down syndrome, intellectual disability (IDD), cerebral palsy, stroke, hospice, age related mobility decline Providing opportunities to optimize independenceEncourage and foster independence while providing necessary supports.Develop professional, supportive, and trusting relationships with clients Manage behavioral needs with compassionPositive Behavior Interventions and Supports (PBIS)Assisting individuals in de-escalation strategies to help with self-regulationPhysical intervention (restraints)Management of behavioral episodesOutbursts, aggression (verbal and physical), elopement, anxiety, Depression, self-injurious behavior (SIB) DocumentationDaily shift notesABC behavioral data collection (Antecedent, Behavior, Consequence)Appointment recordsRequirements Spanish speaking individuals are encouraged to apply. Strong interpersonal skills; empathy for others; demonstrated commitment to serving those with significant needs; ability to work well within a team model and independently. Strong communication skills, including verbal and non-verbal Knowledge of common behavior interventions Driver’s license and Personal vehicle for work useWhen a transportation van is unavailable you will be expected to transport individuals in your own vehicle. The following certifications are preferred. If you do not have any of these CHD offers training at no charge.First Aid and CPRHands on training component requiredNot accepted certifications (Basic, BLS)Medication administration certification (MAP)MAP certification is required to be completed within 6 monthsSafety Care (QBS) All candidates must complete and pass background record screening process.
